REFERRAL FORMS & REGISTRATION
Once registered with Medema Endodontics, doctors and their staff can click on doctor login to refer patients using our secure server. This will allow you to view treatment notes and reports, as well as a complete history of all patients who have been referred. An automatic email will be sent to you when a new report has been posted.
You can download extra referral forms. If you run out, please contact our office and we will get a new set to you as soon as possible.
You can print a Registration Packet for your patient if you would like to send one with them or if they are unable to print them.

Cone Beam Computed Tomography
Many times, a 3-D image can provide extra information that is critical to appropriate treatment decisions.
This information may be necessary for the following procedures:

- Endodontic Surgery
- Root Canal Therapy
Furthermore, if we find out that a tooth cannot be saved, the 3-D image can be used in planning for an implant.
Medema Endodontics is proud to offer 3D imaging services within the office using the Morita Veraviewpocs 3D R100 system.
Global Operating Microscope
All treatment is completed using an operating microscope. This allows Dr. Medema to work with significant magnification and illumination.
Photographs can also be taken for communication with both patients and referring doctors.
Digital Radiography (X-Rays)
Digital x-rays are a great advancement in the field of dentistry. They provide excellent image clarity and significantly reduce patient exposure to radiation. Furthermore, digital images allow for excellent communication with other dentists.
The Gentlewave uses fluid dynamics and broad-spectrum acoustic energy to clean an disinfect the root canal system. It can help greatly with anatomy that would otherwise be difficult to treat. This is a necessary tool to have in the practice for the correct clinical scenarios.
